About the IIRHSE Program
Root cause analysis turns a single incident into a permanent fix, provided the method is applied properly. The program teaches a disciplined investigation sequence and the main analysis tools: five whys, cause and effect diagrams, fault trees, change analysis and barrier analysis. Participants practice on real events, test their conclusions and write corrective actions that survive review. Delivered in-house at the client's premises or live virtual for operations, quality and safety teams.
